1. Telecommunication License Application Form (Letter)

Note

Issue and renewal of Fishing Boat DSB License

1. Same as 1.1-1.3 and 1.5 in 9.1 for license issue.
2. Renewal
2.1 DSB only: Submit the old license to renew the license
2.2 With installation of new apparatus: After the survey is passed, same as as 1.1-1.3 and 1.5
     in 9.1 for renewal and the old license shall be recovered.

Registration for Amateur Radio Personnel Qualification Test by appointment

1. Only group registration will be accepted (no need of appointment for individual candidates
   who may walk in and take the test. The test falls into 4 levels: test for Level 4 is given on
   Mondays and Levels 1-3 on Fridays).

Issue of Dedicated Taxi Radio Station (Ground Station) Establishment Permit

1. Pursuant to the letter of agreement transferred from the motor vehicles authorities
2. Submittals shall be subject to the Administrative Regulations on the Establishment and Operation of Dedicated Radio Stations for Taxicabs. Applicants shall submit relevant documents to the motor vehicles authorities for review.

1. An important permit shall be issued for the following five purposes
1.1 Telecommunications equipment imported overseas for self-use of individuals, corporations and non-corporations.
     (1) Import Permit Application Form
     (2) ROC Citizen ID Card (individuals) or registration certificate of corporations or non-corporations (original or 1 photocopy)
     (3) List of Controlled Telecommunications Equipment
     (4) Bill of Lading (original x 1)
     (5) Catalogue or specification data (original or photocopy x 1)
1.2 Import of non-Taiwan made controlled telecommunications equipment by reshipment (return)
     (1) Import Permit Application Form
     (2) Operation permit or license (original or 1 photocopy)
     (3) List of Controlled Telecommunications Equipment
     (4) Catalogue or specification data (original or photocopy x 1)
1.3 Controlled telecommunications equipment for display, R&D, testing, certification; or spare parts for maintenance, import processing maintenance or export after assembling.
     (1) Import Permit Application Form
